Grammatical Errors by George R.R. Martin

Is it just me? Or is it really there? I seem to find occasions where commas are missing (could be just me) or, in this case, a space is missing (and maybe a comma between [door] Y [she]?): “Outside his door he found Aggo fitting a new string to his bow by the light of a swaying oil lamp.” The above quote is from p. 309, Storm of Swords.
