What makes a series mature or clean?

Just to clarify, I’m not new to writing or books, but one thing has always confused me: what’s the difference between an R-rated series and a PG13? Take Mistoborn for example, it shows hundreds of slaves executed and thousands killed in battle, but it doesn’t have any sex scenes or swearing, so it makes it PG13? First Law is full of violence, swearing, and explicit scenes, which is why it is classified as “grimdark”, but in my opinion, the story is less grim than Mistborn. Is swearing enough to make an R rated book? Is it the sex scenes or just the story? Some sorting rules would be very welcome. Thanks.
